When you request a general conversation, we collect the name, email address, phone number if provided, and general request you choose to send.

How we use it

We use this information only to respond to your request, coordinate a conversation, and protect the website from spam or duplicate submissions. We do not sell contact information.

How contact requests are delivered

Accepted website requests are sent by email to the existing Scoliosis Correction Centers inbox used by the office. Ordinary email and this public form are not medical-record systems, so please limit your submission to contact and scheduling information.

Website measurement

The site uses Google Analytics and Google Ads measurement to understand visits and count accepted contact-form requests. This may include page, referral, campaign, device, and advertising click identifiers. A form event records that a request was accepted; it does not record a diagnosis, appointment, treatment, or medical outcome.

Important health-information boundary

This website form is not a secure medical portal. Do not send medical history, diagnoses, X-rays, insurance information, date of birth, or urgent medical information through it. For urgent concerns, call emergency services or your clinician.

Retention and choices

Contact requests may be retained as reasonably needed to respond, maintain office records, prevent abuse, and meet legal obligations. You may ask the office to access, correct, or delete your contact information, subject to applicable recordkeeping requirements.
