How website contact requests are handled.

What we collect

When you request a general conversation, we collect your name, email address, phone number, the Connecticut or Florida center you could attend, whether you could attend the full 8-day intensive if it is recommended, and any non-clinical scheduling or location question you choose to send. Some general website forms may ask for fewer fields.

How we use it

We use this information only to respond to your request, understand location and scheduling fit, coordinate a conversation, and protect the website from spam or duplicate submissions. We do not sell contact information.

How contact requests are delivered

Accepted website requests are sent by email to the existing Scoliosis Correction Centers inbox used by the office. Ordinary email and this public form are not medical-record systems, so please limit your submission to contact and scheduling information.

Website measurement

The site uses Google Analytics and Google Ads measurement to understand visits and count accepted contact-form requests. This may include page, referral, campaign, device, and advertising click identifiers. Your preferred center and answer about 8-day attendance are included only in the office email and duplicate-submission check; they are not added to Google Analytics or Google Ads event details. A form event records that a request was accepted; it does not record a diagnosis, appointment, treatment, or medical outcome.

Custom contact-form attribution

For the website's custom contact-form attribution, your browser stores campaign source, medium and name, advertising click identifiers, the landing-page path and the date the record was first saved. Records that are at least 90 days old are no longer used and are cleared or replaced the next time the website reads them, including before a form submission. This is not a background deletion process while the website is closed. The custom form submission sends the current-page path and the referring site's origin, without their query strings or fragments. This period applies to the custom browser attribution record; office contact records and Google's separate measurement records have their own handling.

Important health-information boundary

This website form is not a secure medical portal. Do not send medical history, diagnoses, symptoms, X-rays, insurance information, date of birth, treatment details, or urgent medical information through it. For urgent concerns, call emergency services or your clinician.

Retention and choices

Contact requests may be retained as reasonably needed to respond, maintain office records, prevent abuse, and meet legal obligations. You may ask the office to access, correct, or delete your contact information, subject to applicable recordkeeping requirements.
